I hope you find looking at documentation examples to be helpful. The closest report we have to what you are looking to write is probably a eval/justification for a power wheelchair.

Hello, I’m a COTA. Glad to be a part of the group and I’m looking forward to learning here.
My name is Darlene and I’ve been a COTA for 22 years! I have worked in pediatric outpatient for the last 13 years and prior to that I was a school based therapist and also worked in EI. I’m working on getting a feeding specialty certificate and so inspired by the Maternal Mental Health podcast I’m re-thinking my goals! I’m 60 this month, and, no lie, it is getting harder and harder to get down on gym mats and push #60 kiddos on the swing!

#26: Maternal Mental Health and OT with Dr. Carlin Reaume
My Passion is nature and I love nature based play. It is difficult to incorporate in the setting I’m in but I do what I can. I’m one of those people that are curious about everything and have a hard time focusing and mastering any 1 thing. At the moment I am having the most difficulty with time management . I see kids back to back from 7:45 in the morning to 2:30 in the afternoon I try to do my notes duri g my 1/2 hour lunch. I literally have no other time for it. My supervisor is so understanding and is feeling the strain as well. I can complete point of service notes with less than half of my case load but the rest of them are so hands on there’s just no way I can do that and keep them safe orkeep them focused. I did try verbally dictating my notes and I felt like all I was doing was correcting errors. I’m not complaining, I do love what I do…I love my students. I am grateful to have my caseload. I just really need to figure out the situation I am in!
Ha! You are like me and perfect for the Club! I am also curious about so many things! And, was convinced OT needed a generalist learning platform after reading the book below! Our world is so complex and interconnected- I think it is the super power of OTs to have a broad base of knowledge. Just like your in your case your passion for nature probably makes you a better school therapist!
I also hear you on the struggle of writing concise but impactful notes. I am also very interested in that and will continue to explore that in the Club!

Hello OT Potential Club!
I am an OT with a private practice specializing in pain rehabilitation and have been presenting at OT conferences and interdisciplinary conferences on current pain science and pain therapy interventions both nationally and internationally for 8 years. I am a contributing author for the updated AOTA position paper on pain management, and started the PainOT Facebook group (over 1,400 members) in 2018 to support OT practitioners in understanding the most up to date evidence and practice for pain - both acute and chronic.
In addition, I am the current president of the OT Association of Colorado and am actively involved in advocating for our profession. I have over 30 years of experience in many practice settings with specializations (in addition to pain) in neuro rehabilitation and complex seating and positioning.
I am excited to join this group of practitioners looking for the best evidence to guide practice!

Hi all, I’m Staci; I’m excited to be in the group and have the opportunity to learn and grow. I’ve been a COTA for almost 8 years, in a variety of settings. Currently working with mostly pediatric clients in a setting incorporating Hippotherapy/Equine Assisted Therapy, and am loving it. Excited to expand my knowledge of OT and EBP.
Hi everyone, I’m a new COTA and will be starting work soon in an outpatient peds clinic. I’m super excited but also a little nervous. I have newly diagnosed adhd and am still learning how to manage the symptoms. I hyper focus on research and finding new activities for my clients, so I’m hoping I will find some good EBP here. Oh, and I am not great at writing concise notes, so I’m also hoping to find some help with that lol.
