I first saw this one from @sarah112 in her My OT Spot post…and I think about it whenever I think about documentation 
This is still me with my 10 SNF patients a day! Thank goodness we have paper cards for each one to jot down their assist levels and a quick blurb about what we did right after the treatment!

This is probably my all-time favorite one since I still get SO excited when I meet another therapy friend or anyone who knows what we do!
